Preparing To Go Home

Your rehabilitation team will discuss your discharge day with you and your family. They will ensure that you have the information you need to stay as healthy and as independent as possible, and to be able to continue to work on your goals and build on your progress.

Checklist:
  • Your going home day
    Discharge time is 9:00 a.m., though you can choose to leave as early as 7:00 a.m. In advance of your discharge day or going home day, please arrange with a family member or a friend to pick you up.

  • Medications
    Your care team will have given you a list of prescriptions for any new medications. Please ensure that you understand these medications by speaking with your pharmacist and nurse.

  • Mobility and assistive devices
    Please speak with your therapists about recommendations for mobility aids, assistive devices, modifications that may be need to done in your home, and about any eligible funding. Some devices may be available for purchase from the hospital gift shop. Your therapists can provide you with a list of sources for other equipment.

  • Exercises
    Please speak with your therapists about recommended exercises and activities you should continue 
to do at home.

  • Continuing rehab (outpatient rehabilitation), after you are home?
    Your care team will discuss with you recommendations for outpatient rehabilitation at St. John’s Rehab or at other rehabilitation programs closer to home. Before your discharge day, they will have submitted a referral, and will have more details about this therapy and your first appointment time.

  • Transportation to outpatient rehabilitation
    For transportation to outpatient rehabilitation at St John’s Rehab or a closer-to-home facility, you are responsible for making arrangements. You may be eligible for Wheel-Trans or Mobility Plus services in the community. Please ask your rehabilitation team for information about the application process.

  • Support resources in the community
    Please speak to your care team about home and community supports such as help with meal preparation, homemaking or transportation services. You can also visit the Resource Centre in the Horsfall Eaton Wing Lobby, to gather more information.

  • Bill payment (if applicable)
    Patients can contact Patient Accounts with account inquiries at 416-480-4156 any time between 8:00 a.m. and 4:00 p.m., Monday to Friday. Messages left after hours will be returned the following business day.

  • Please collect all your belongings
    Please collect all of your belongings and hospital cards, and any care instructions and prescriptions you have been given, during your stay.
