Sunnybrook and Humber River Hospital partner on General Internal Medicine Rapid Referrals

Sunnybrook and Humber River Hospital (HRH) are pleased to announce a new partnership. As of Monday, April 4th, patients in the Sunnybrook Emergency Department that reside closer to HRH and would otherwise be referred to the Sunnybrook General Internal Medicine Rapid Referral Clinic, are now referred to the HRH Urgent Care Medical Clinic, when appropriate.

The partnership leverages the new and improved infrastructure at Humber River Hospital, and provides patients from the Humber River area who come to Sunnybrook with less complex care needs service closer to their home.

The Rapid Referral Clinic model, which is used in many hospitals around the world, has been a highly successful way to care for a patient population that requires further investigation into their health concern (e.g. blood testing, medical imaging), but does not require an admission to hospital.
