New partnership helps patients manage health online

A partnership between Central Community Care Access Centre (CCAC) and Sunnybrook will launch this spring with the aim to improve access to information and continuity of care processes between inpatient and outpatient community services. Sunnybrook's innovative MyChart™ system will be used to link the two organizations.

"MyChart™ is an online system that allows users to travel freely between health care providers with easy access to critical information," says Sarina Cheng, Director, eHealth Strategies & Information and Telecommunications Operations at Sunnybrook. "It provides easy access to educational material, personal diaries for self monitoring, medication history, test dates, test results all in one record that is set up, managed and stays with the consumer."

Central CCAC services 75,000 clients annually with 2,000 transitioning through Sunnybrook. With MyChart™ Central CCAC will be able to provide timely access to information to its staff, patients and clinicians. For example, Central CCAC patients will have online access to case manager contacts, placement information, appointments, educational material, clinical and personal information.

"With MyChart™ clients have access to their health information whether it is being delivered in the hospital or the Central CCAC," says Cathy Szabo, Chief Executive Officer, Central CCAC. "This is an important step forward in building a seamless health care system and ensuring clients have the tools to enable them to actively participate in their care."
