Integrated Community Program

Our Integrated Community program stems from an innovative concept developed during our collaborative 2018-2021 strategic planning process.

For many patients, Sunnybrook is one of several stops on a care journey, and we’re committed to ensuring that the transition from hospital to the next step is one where everyone involved is both informed and engaged.

The integration of our emergency department with community and family medicine compels us to continue to build and sustain partnerships with fellow hospitals, nursing homes, and other health care providers to ensure patients don’t get ‘lost in the system’. Moreover, this program seeks ‘wrap around’ care for patients to ensure consistency in quality and access to services across the system of care.

As we continue to develop this new integrated program, we will seek to improve access to the specialty care services we have available by partnering with family doctors in the hospital’s neighbouring community and with teams in other institutions across the province.

Recognizing the importance of sharing information with a patient’s health care professionals in the community, we’re committed both to improving how information is provided and to ensuring the institution is as accessible as possible to community providers for referrals.
