Welcome to Sunnybrook EP Remote Monitoring Clinic

Your Connection to Advanced Cardiac Care

At Sunnybrook EP Remote Monitoring Clinic, we are dedicated to enhancing cardiac care through innovative remote monitoring services. Currently focusing on Implantable Cardioverter Defibrillators (ICDs), and with future plans to include Implantable Cardiac Monitors (ILRs) and Pacemakers (PPMs), our clinic integrates the latest technology to keep patients connected and cared for.

Our Mission and Values at Sunnybrook

Our team of cardiac specialists are committed to delivering high-quality remote care aligned with clinical standards. We prioritize patient well-being, aiming to provide continuous, effective monitoring that supports optimal health and peace of mind.

What is Remote Monitoring?

Revolutionizing Cardiac Care

  • Increased Comfort: Enjoy the reassurance of continuous monitoring and timely support from the Sunnybrook EP team (All emails will be answered within 72 hours, kindly refrain from sending multiple emails).

How It Works

Remote monitoring is an essential complement to in-clinic visits. It enables ongoing observation of your ICD's performance and immediate response to alerts, enhancing patient care by integrating with standard clinical practices to provide real-time, actionable insights.

Integration with ICDs

ICDs continuously track heart rhythms and deliver life-saving interventions when necessary. Our remote monitoring system enhances this process by allowing for real-time data analysis, improving response times, and ensuring personalized care adjustments.

Eligibility

Who Can Benefit?

Our remote monitoring program is available to all patients with ICDs. Currently, our services are focused on ICDs, with future expansion planned to include ILRs and PPMs.

Conditions Monitored

ICDs are primarily used for managing various heart conditions. For detailed information on these conditions, please refer to the My ICD Guide.

Enrollment Requirements

Enrollment details and specific criteria will be discussed during your initial consultation at Sunnybrook. Our team is here to guide you through each step of the process.

Importance of Ensuring Your ICD Transmitter and Device Are Connected: Maintaining a reliable connection between your ICD transmitter and device is crucial for effective remote monitoring and optimal cardiac care. Ensuring this connection enables the following benefits:

  • Real-Time Data: Seamless transmission of heart rhythm and device performance to healthcare providers.
  • Enhanced Management: Timely updates and adjustments to device settings based on accurate data.
  • Advanced Analytics: Accurate data collection for better predictive insights and early interventions.
  • Improved Outcomes: Early detection of potential issues leads to quicker medical responses.
  • Fewer Visits: Reduces the need for frequent in-person check-ups, enhancing convenience.
  • Personalized Care: Tailored treatment plans based on real-time, reliable data.

By ensuring that your ICD transmitter and device are consistently connected (plugged in and turned ON), you support enhanced care, timely interventions, and a more streamlined management of your heart health.
