Care Navigation Team
The Care Navigation Team assists the patient and/or family in navigating the healthcare system.
Chronic Disease Self-Management Program
If you would like to participate in a 3 week ( 2 ½ hours each week) self- management program focusing on living with a chronic condition, please call 416-480-4805.
Diabetes Education Program (SUNDEC)
A team of healthcare professionals who specialize in teaching and supporting people with diabetes.
Dietitian and Nutritional Counselling
Our dietitians provide a wide range of counseling services including: nutrition across the lifespan, heart health and more.
Homebound Seniors Program
A program that provides team-based care to patients who are homebound and have difficulty accessing the clinic.
IMPACT Clinic (Interprofessional Model of Practice for Aging and Complex Treatments)
A comprehensive interprofessional team dedicated to improve chronic disease management.
Nurse Practitioner
The nurse practitioner has an expanded scope of practice encompassing comprehensive assessment and diagnosis, ability to order and interpret diagnostic tests, prescribe medications and other treatments, and make referrals to specialists.
Occupational Therapy
The occupational therapist offers assessments for patients with chronic conditions or disability around activities of daily living with a goal to assist patients to optimize function and quality of life.
Pharmacy Services
The pharmacists offer medication reviews, working with patients and/or their caregivers to support the safe administration and management of multiple medications. Plus: see our smoking cessation tips & strategies.
Primary Care Nursing
Your nurse works in partnership with you and your family physician. Registered primary care nurses with experience in assisting families to stay healthy and supporting those with acute illness and chronic conditions are usually involved in some aspect of your visit, such as history taking, health teaching and/or performing procedures. Your nurse is also available to offer advise over the telephone during office hours.
Procedures Clinic
In this clinic, small procedures are conducted by residents under direct supervision by a clinic physician.
Psychiatry Shared Care
A program that delivers high-quality psychiatric care while at the same time providing for learning opportunities for ourselves and our residents.
Social Work and Counselling
Our social workers provide counseling and support for a variety of issues, including anxiety, depression, grief and bereavement.
Clinic hours (By appointment only)
Regular office hours: Monday-Friday 9am-4:30pm
Evening hours: Monday-Thursday 5:30-8:00pm (after hours physician)
Weekend coverage: Saturday and Sunday 9am-3pm (after hours physician)
*Weekend on-site clinic times will vary depending on the week and are strictly by appointment only. If you need after-hours assistance, please call your family doctor’s regular office number for information on how to connect with someone for help for urgent matters.
