Obtaining your patient identifier number

If you have provided an email address or cell phone number to your surgeon's office, you will receive a text message and email one business day before surgery with your unique patient identifier number. Your identifier number will only appear on the tracking board after you have registered for your surgery.

If your patient identifier number has been lost, please call 416-480-6100 ext. 64240 and speak to the patient administrative associate to obtain your number.
