Outpatient referrals are welcome from physicians, rehabilitation consultants and WSIB case managers. Please ensure all related documentation is included for the application to be processed.

Some programs feature specific admission criteria; please review to ensure your patient is eligible for the requested program

Community Rehab Program Admissions

Each program operates its own intake process. Most require physician approval. Please review specific application requirements

Wellness Clinics

Our wellness clinics do not require a physician referral. Appointments can be made directly with the clinic
