Toolkit: Transitions in mental health and addictions care for youth

Transitional-aged youth, typically considered to be between 12 and 29 years of age may face challenges accessing appropriate support resources as they transition out of child services.

This Toolkit contains a series of downloadable information sheets for youth, caregivers, service providers, and decision makers for insight on how to prepare, and what can be expected, while accessing mental health and addictions care during the transition stage.

These materials are based on research conducted by FNP and our partners from 2019-2021 (Study 1 and Study 2). References are available in the downloadable toolkit.
