Back on Track - Motor Vehicle Collision

How Can We Help?

Our experienced and knowledgeable team of health-care professionals will work with you to create a customized treatment plan that will help you reach your goals and get back to the meaningful activities that were impacted by your injuries.

Services may include:

  • Intensive physiotherapy and occupational therapy
  • Counselling and support from social work and psychology
  • Complementary therapies such as massage, hydrotherapy, acupuncture and chiropody
  • Customized prosthetics created on-site in our dedicated lab
  • Access to chiropractic and pressure garment services
  • Coordination with your auto insurance company

Some services may be available virtually.

Cost & Coverage

The Motor Vehicle Collision Program is covered by your auto insurance company and extended health insurance plan. To access our services, you must have an open auto insurance claim and provide consent for services to be billed directly to your auto insurance company.

All auto insurance policies in the Province of Ontario stipulate that extended health insurance benefits, must be billed prior to billing auto insurance. The auto insurance is secondary and will pay the portion not covered by your extended health insurance benefits (if applicable).

Referral Information

A referral from a physician or rehabilitation consultant is required. Patients must have appropriate rehab goals related to traumatic injuries resulting directly from a motor vehicle collision. Injuries must include at least one fracture caused by the collision. Complete and fax our referral form to 416-226-3358.

Once we receive your referral, our service coordinator will contact you and begin to work with you and your health care team to confirm your auto insurance claim details. Upon starting your rehab, you will meet again with our service coordinator to review the process for submitting treatment plans, billing, and communicating with your auto insurance company.
