
Welcome to the Family Practice Division of the Department of Family and Community Medicine.
Our Family Practice Unit provides a broad range of primary care services to patients and families across the life span. Low-risk maternity care, pediatric, menopause, chronic disease management and complex care of the elderly are some of the foci of care.
Our mandate includes teaching as part of the University of Toronto’s Department of Family and Community Medicine and research through the Primary Care Research Unit (PCRU) to improve knowledge and skills in the practice of Family Medicine.
Our Family Practice care team includes family physicians, resident physicians, medical students, nurses, a social worker, dietitian and several diabetes educators.

As per our agreement with the Ministry of Health and Long-Term Care as a Family Health Organization (FHO), patients must reside within the following boundaries:
- Sheppard Avenue to the north
- Victoria Park Avenue to the east
- St. Clair Avenue to the south and
- Bathurst Street to the west
Patients will be seen in the clinic by family practice residents and medical students supervised by faculty physicians. Other members of our health care team including a social worker, dietitian, diabetes educator and/or primary care nurse may participate in your care as needed.
