Welcome to the Sunnybrook Academic Family Health Team (SAFHT)
The SAFHT provides a broad range of primary care services to patients and families across the life span. We are committed to caring for our patients and their families through collaborative teamwork, health promotion, disease prevention, system navigation, mental health programs and chronic disease self-management in order to provide excellent care for all. Low-risk obstetrical care, pediatrics and complex care of the elderly are some of the foci of care.
Our mandate includes teaching as part of the University of Toronto’s department of family and community medicine and research through the Primary Care Research Unit (PCRU) to improve knowledge and skills in the practice of family medicine.
As an Academic Family Health Team, education and teaching of healthcare professionals are a large part of our role. This includes supporting continuing education and professional development opportunities for all staff. In addition to seeing their assigned family physician, patients will also have clinic visits with a family medicine resident or medical student, supervised by faculty physicians.

Clinic hours (By appointment only)
Regular office hours: Monday-Friday 9am-4:30pm
Evening hours: Monday-Thursday 5:30-8:00pm (after hours physician)
Weekend coverage: Saturday and Sunday 9am-3pm (after hours physician)
*Weekend on-site clinic times will vary depending on the week and are strictly by appointment only. If you need after-hours assistance, please call your family doctor’s regular office number for information on how to connect with someone for help for urgent matters.
